This patient had a previous Septoplasty and Rhinoplasty done five years previously as well as filler two years prior. Filler was dissolved prior to surgery. The septum was revised and ear cartilage was used for the nasal reconstruction. A double spreader graft was placed on the right in a single was placed on the left. Alar rim graphs were placed for both nostril support and contour correction and the shape of the tip was repaired. Many patients are concerned about grafting and they believe it will widen the nose. Nasal grafting in Rhinoplasty is done mostly for support. The overall shape of the tip is determined by surgical tecnhique and surface contour grafts, which are small and soft. A contoured nose doesn't have to be small, it just has to have the right shape. Many revisions are the result of shape issues, not size problems.
