Full Case Details:
For this revision patient, the key issue was that her nostrils were too open. We used tip reconstruction, including a columellar strut, a cap graft and alar rim grafts to lower the nostril edges and support the nasal sidewall, since the cartilages had been transected in the prior surgery. We also shaved the bottom of the septum, allowing compression of the tip, rather than rotation. The profile was shaved 1mm and a left spreader graft was placed.
Restoration of non-angular nasal transitions is a common requirement for revision rhinoplasty. Understanding the interplay of light, soft tissue, and underlying structure is critical in secondary cases. Excessive removal of structure without creating alternative support is often the reason the revision is required. Replacing that support with an eye towards artistry provides natural revision results.
