Full Case Details:
Problems with the infratip are some of the most common issues leading to revision surgery. Having a droopy tip can throw off overall nasal aesthetics, leading to an undesired outcome. Infratip fullness usually exists with dissatisfaction during smiling views, which was present in this case. Creating the right shape for the tip can dramatically alter the overall appearance of the nose, without causing extreme shortening or over-rotation of the nasal tip.
After a previous open Rhinoplasty, the nose was reconstructed with high spreader grafts on both sides, Tip sutures, and multiple grafts, including a Columellar Strut, a Cap graft, and bilateral Rim Grafts. Intermediate crural excision (or Vertical Lobular division) was done in order to avoid over-rotation and over-projection. Temporalis fascia was placed over the bridge to provide enhanced contouring.
