Full Case Details:
4TH REVISION RHINOPLASTY. This patient originally saw me in 2016 but elected to have surgery elsewhere. He then underwent four nasal procedures (two closed and 2 open rhinoplasties). The patient felt that the prior surgeons had not listened to his goals and, therefore, didn't give him the type of nose that he was trying to achieve. There was also asymmetry and persistent deviations that were bothersome. In order to ensure effective communication, we went through an extensive preoperative consultation process. We discussed the risks and potential limitations of a fifth nasal surgery and the patient understood. Using cadaveric donor rib, temporalis facia, and materials found in the field during surgery, we were able to provide him with the results he had been seeking for 10 years. We remodeled the bony side walls for better contouring and symmetry. We shaved irregularities and lowered the profile 1.5 mm. We reconstructed the tip with a columellar strut and 2 cap grafts. We did a 3mm nostril reduction, using a different technique on each side to establish better symmetry. We placed temporalis down the middle of his bridge to provide a smooth results and central contouring. I am happy to say this patient's rhinoplasty journey is now over.
