Full Case Details:
This patient was interested in facial rebalancing so rhinoplasty and an auto fat chin implant were performed. A piece of fat was removed from underneath the chin and placed into the chin area as a small implant. The patient wanted moderate nasal appearance changes so the profile was reduced, and the bones were remodeled. Were replaced spreader grafts on both sides, thicker on the left and the tip cartilages were reshaped. A columellar structure was placed using cadaveric rib. Alar rim graphs were also placed and three layers of cap grafts replaced in the center. Donor rib was used in this case because Asian patients often have only a small amount of septal cartilage that can be removed for rhinoplasty purposes. 1 mm was removed from the left nostril to improve symmetry.
Rebalancing the profile during a Rhinoplasty can also involve chin surgery. In cases where patients need less projection than implants typically provide (usually 3mm or less), fat can be used as an alternative. Fat is harvested and placed in the same manner as an implant through a small incision under the chin. This is different from fat transfer, which involves harvesting, processing, and injection. Free fat grafts retain more shape and are sometimes preferred in chin augmentation.
