Full Case Details:
A pushing philtrum describes a situation in which the upper lip elevates as it approaches the based of the nose. This is often due to an enlarged bony projection in that area or a long septum. Trimming this results in improved upper lip aesthetics and also assists with nasal tip deprojection. After reduction of the anterior nasal spine and shaving the caudal septum, no additional deprojection maneuvers were required in this case. Profile reduction, septoplasty, a left spreader graft, and tip-plasty were performed. A columellar strut and alar rim grafts were used, along with a cap graft, to reshape the nasal tip. The overall result is very balanced, despite the significant degree of physical change seen in this case.
