Full Case Details:
After a previous unsuccessful Septorhinoplasty, this patient was concerned with breathing obstruction, a droopy tip, and cartilage asymmetries that were extremely visible. She didnt want her nose to change size substantially. Temporalis fascia was used to cover her tip and bridge, due to very thin skin. The profile was conservatively lowered for better balance and the septum was revised. The tip cartilages were reshaped and stabilized with a columellar strut after being shortened with vertical intermediate lobular division. A cap graft was used to further soften the nasal tip cartilages and prevent visibility and batten grafts were placed to limit narrowing of the tip. This case demonstrates what we call the "Shoe Horn Effect" - when cartilages are too long and not stabilized with grafts and sutures, they tend to pop out in areas that are weak, creating bumps and deviations.
