Full Case Details:
Closed tip and profile deviation correction and cosmetic rhinoplasty with chin implant. A pushing philtrum describes a clinical situation in which the upper lip elevates as it approaches the based of the nose. This is often due to an enlarged bony projection in that area or a long septum. Trimming this results in improved upper tip to lip aesthetics and also assists with nasal tip deprojection. Rebalancing with a profile reduction and chin implant was also essential in this case. Deviation correction was accomplished with septoplasty, a right spreader graft, a left subalar graft and tip surgery. Sutures were placed into the tip cartilages to correct length asymmetries and a columellar strut was placed for long term support.
